About the event
By filling out this online registration form, you hereby give permission for your child to attend the Stripes Kids Camp. You give permission for your child to be transported in a privately owned vehicle where required. You hereby authorise the person(s) in charge, where it is impractical to communicate with me, to arrange for my child to receive medical or surgical treatment as may be deemed necessary.
